Medical + Dental + BehavioralServes American Indian and Alaska Native people in Maricopa County, including those experiencing homelessness. Medical, dental, behavioral health, and substance use — sliding-fee scale, no cost for uninsured patients who qualify.

Ages 18–24Primary provider for young adults experiencing homelessness in Phoenix. Dorm-style emergency shelter, drop-in center (M–F 7am–4pm) with meals, laundry, showers, and case management, plus transitional living and rapid rehousing. A Youth Coordinated Entry Access Point — standard CE processes are often inappropriate for youth, so HomeBase operates the youth-specific pathway.
YOTO — Youth on Their Own
Under 24Supports unaccompanied youth and young adults experiencing homelessness. Housing placements, case management, and stabilization services not available through adult programs.
